Our team is seeking an experienced nurse or Allied Health professional (with AHPRA registration) to deliver goal-directed, early intervention tele-health services including care planning, health coaching, and support coordination with a highly engaged patient population. We are looking for both 12-month fixed term and permanent full-time employees. You will have the opportunity to work flexibly as the role can be hybrid working arrangements.

Job Description Tasks include: Support and care for clients with empathy and expertise to improve their personal health and wellbeing goals. Undertake coordination of supports and wellbeing services for a highly engaged and motivated client population. Provide initial early intervention triage with clients, supporting both physical and mental health services. Undertake relevant case coordination activity including liaison with all stakeholders; provision of email updates; and creation of appropriate documentation such as treatment plans or other documentation as required. Qualifications Requirements/Qualifications Experienced Nurse or Allied Health practitioner with full registration. Mental health support service experience highly desirable. Caring and empathic nature. Experience in care coordination or occupational rehabilitation. Well-developed written and verbal communication skills. Good IT and time management skills.
