Team HEALTH has been providing services to people with mental illness in the Northern Territory for over 30 years. Team HEALTH was established by a group of relatives and service providers of people with a mental illness who wished to provide stable, safe accommodation for their family members. Team TALK is a low intensity psychosocial phone-based support line established for Territorians. Following the PHN Low Intensity Mental Health Services (LIMHS) for Early Intervention Guidelines, Team TALK is funded by the Northern Territory Primary Health Network (NTPHN). Reporting to the Coordinator, the Team TALK Coach provides phone-based support to participants utilizing an early intervention and psychosocial approach. The Team TALK Coach will focus on providing supports that are person-centred, recovery-focused, strengths-based, timely, culturally appropriate, and assist callers in navigating supports and services at Team HEALTH and in the wider community. The Team TALK Coach will be responsible for ensuring quality phone-based supports to both one-off callers and ongoing scheduled participants. Coaches will typically monitor/support participants for up to six weeks through their mental health journey. The Team TALK Coach will utilize a range of tools including the Team HEALTH Stay Strong plan, a specialized Individual Recovery Plan (IRP) that enables participants to set goals. IRP goals support the development of skills to maintain independent living, self-management of one's mental illness, and service access in the community. To be successful in this role, you will require: Availability for shifts: Monday & Tuesday 5-8pm, Wednesday & Thursday 9am-5:15pm, which are the required hours for this position. Appropriate qualifications in community services, mental health, or a related area. Demonstrated understanding of the recovery model and psychosocial rehabilitation. Demonstrated experience in the delivery of services to people with mental illness and psychiatric disability in a community setting. Demonstrated high level of communication skills (written and verbal), with the ability to interact with a diverse range of people. Action-oriented, flexible, and innovative approach. Demonstrated computer literacy in Microsoft Office, and the ability to use and learn new systems, such as Client Management Systems. At times, the Team TALK Coach may be required to work across programs within Team HEALTH.
