OUR OPPORTUNITY We have an exciting opportunity for a Team Leader Youth Case Management and Better Futures to join our Out of Home Care Team. Cafs provides high quality support services to young people, including high risk children and adolescents. Youth Case Managers support case contracted young people who reside in our residential homes, as well as young people supported by Targeted Care Packages (TCP) to remain safely with family or in the community. TCPs provide opportunities for young people currently residing in residential care, or at risk of entering residential care, to enter a more suitable environment such as returning to family, entering foster care, or transitioning into independent living.
Better Futures Practitioners provide practical support to young people through our Better Futures and Home Stretch programs, as they transition from Out of Home Care to independent living. These programs engage young people early supporting them to have an active voice in their transition planning, and providing individualised supports both in-care and post-care across a range of life areas including housing, health and well- being, education, employment, and community and cultural connections.
The Team Leader will lead and supervise a team of Youth Case Managers and Better Futures Practitioners in providing high quality case contracted and support services to young people, including high risk children and adolescents. The Team Leader Youth Case Management & Better Futures works collaboratively across Youth Care and Out of Home Care programs and works closely with Cafs Therapeutic Lead to ensure the provision of consistent, high quality and (TBRI) focused therapeutic support who reside in a Cafs residential care unit or are supported by a TCP, Better Futures, or Home Stretch funding or an alternate funding mechanism.

Key skills and attributes:
You have a strong desire to deliver good outcomes for young people. You have demonstrated knowledge, skills, and experience in trauma informed principles and approaches. You have substantial experience in reflective practice Your highly developed cultural awareness and sensitivity, partnered with excellent communication skills, will enable you to build strong relationships, with our stakeholders. Alignment to Cafs values of Respect, Integrity, Collaboration, Kindness & Innovation MANDATORY QUALIFICATIONS Tertiary Diploma or above level qualification in Social Work, Welfare, Community Services, Psychology or other relevant field.
