The RAHT is an established and progressive multi-disciplinary team which includes Dietetics, Occupational Therapy, Physiotherapy, Podiatry, Social Work and Speech Pathology services. The team assists people with a disability and older frail people and their carers, providing interventions in predominantly home-based settings, making a true difference to our clients' independence and well-being.
The Team Leader & Quality Co-ordinator will provide various support functions within the RAHT including:
Managing a small caseload Supporting the day-to-day operations of the team Co-ordination and support of the intake of referrals Monitoring team key performance indicators Supporting human resource functions and staff development Supporting quality initiatives and service development and continuous quality improvement Ensuring the delivery of services is in accordance with the CHSP, HACC PYP and NDIS program guidelines The position will also make an active contribution to the ongoing development of the RAHT including involvement in quality reviews and accreditation processes. You will report to the Rural Allied Health Team Program Manager whilst providing high quality health services, consistent with your professional body code of ethics and practice standards. A comprehensive orientation program is available.
We are looking for a Team Leader & Quality Co-ordinator who:
Current registration with the AHPRA, where required. Has experience in the delivery of Allied Health Services within home, group and community settings to consumers with complex presentations, inclusive of complex health conditions, memory impairment, cultural and diverse backgrounds. Is self-motivated and self-directed with a high level of communication, organisational and time management skills. Has experience in undertaking projects, continuous quality improvement initiatives and accreditation processes. Has an ability to demonstrate sound leadership and work both independently and as a member of a team. Holds a current motor vehicle licence.
