The Team Leader, NDIS Services plays an important hands-on role in the planning and development, implementation, review and evaluation of a person's Individual Person-Centered Plan, whilst also providing supervision, mentoring and development to staff.
Located in Muswellbrook, the role works in Supported Independent Living, Monday to Friday between the hours of 6:00am-8:00pm. This role is approximately 30 hours of hands-on support and 8 hours admin. This is a perfect opportunity for the next step in your Disability Support Work career.

Drive Samaritans' Person-Centred Planning processes, overseeing support delivery, maintaining up-to-date documentation, and providing a consistent point of contact for supported individuals and their families. Assist in identifying and assessing client needs, supervise staff, ensure professional and compassionate service delivery, and manage various personal care tasks. Maintain effective communication and relationships with stakeholders, support clients' living skills development, ensure accurate and timely documentation, report incidents, and contribute positively to teamwork and achieving outcomes. About you: Tertiary qualification in relevant field or commensurate experience Demonstrated experience in the delivery of services to people with a disability. Established coordination and problem-solving skills. Demonstrated knowledge and understanding of NDIA planning processes. Ability to think creatively and use initiative, to work independently or as part of a multidisciplinary team. Ability to effectively develop and maintain positive working relationships and networks with government and non-government agencies and services. High level interpersonal skills that support positive and productive relationships. Strong written and computer skills for the creation of plans, programs, reports, correspondence and other documents.
