Main duties of the jobAs a Team Leader, you will focus on supporting the home management team in delivering high-quality care with a difference. Your responsibilities will include leading in the person-centred care planning process, assisting with resident discharges, monitoring resident wellbeing, and practicing safe systems of work. You will need to have an NVQ 3 or equivalent in care, two years' experience in a senior position, and the ability to communicate effectively and lead staff.

Job responsibilitiesYour focus as a Team Leader will be to support the home management team in delivering high-quality care by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.
Other responsibilities will include:
Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.Monitoring the well-being of each resident, being aware of any change impacting care and care planning, and ensuring that changing needs are met and written records are amended to reflect any variation.Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.About youTo succeed in your application, you will live our values of caring, supportive, honest, respectful, and accountable in all you do.
Our ideal candidate must:
