Job DescriptionThis is a newly created position at Hannah's House, where we are looking for a Team Leader – Family & Carer Support working on a Part-time basis 38hrs a fortnight to join our friendly team, based in our Osborne Park Head Office.
This is an initial 12-month contract.
The Team Leader – Family & Carer Support will establish and maintain the Family & Carer Support Service involving volunteers providing support and practical assistance to families caring for a child with a disability.
The Family & Carer Support Service provides links to the community by supporting families with referrals to other required providers, as well as internal referrals to play programs and family events at Hannah's House.
Reporting to the Director of Clinical Services, responsibilities will include but not be limited to:
Families and Carers Assists families and significant others such as parents and carers in accessing relevant agencies and community resources.Engages communities of interest to support families and children.Delivers a Family & Carer focused service which consults with and involves families & carers in service planning, delivery and evaluation to assist in continuous improvement.Provides ongoing support and assistance to children and young people via the Play Program.Engages with children and young people, their families and carers to ensure their safety and wellbeing.Adheres to Hannah's House Child Safety & Wellbeing Policy in relation to the appropriate treatment of children.Leadership Leads, motivates and coaches the team, ensuring successful achievement of service objectives.Leads by example and models appropriate behaviour to help build a positive workplace culture.Administration and Analysis Ensures that all families have a valid and current Service Agreement, and that changes and updates are actioned in a timely and efficient manner.Monitors and ensures accurate and timely documentation within the CRM for all families.Safety and Quality Upholds the principles of a child safe organisation and ensure that the protection and safeguarding of children is of upmost priority.Takes reasonable care for their own health and safety and for others and participates in maintenance of a safe work environment as per Hannah's House Health and Safety Policy.Desired Skills and ExperienceMinimum Certificate IV in Community Services, Minimum Certificate IV in Case Management, or other relevant Certificate IVTransferable customer service or management experienceDemonstrated ability to effectively manage complex situations and think criticallyProven experience in supervising, developing and mentoring staff & volunteers Highly developed interpersonal, oral and written communication skillsProficient use of IT systems and processes including Microsoft Suite, Client Management Systems and Rostering SoftwareHigh level ability and experience in planning, organising, and achieving work prioritiesKnowledge and understanding of the National Disability Insurance Scheme (NDIS)Relevant experience in the delivery of support services in the disability and/or health sectorAs a child safe organisation, we have zero tolerance for any form of child abuse.
