Chakra Care is a disability support service in WA. Our exceptional team supports families, youth and elderly in the community. We work closely within multidisciplinary teams of like-minded providers ensuring the goals and needs of our clients are consistently met and turning your hope into a purposeful reality.
Role Description We are seeking an enthusiastic Team Leader to support our clients starting February 2025 to lead a small team active between Joondalup and Two Rocks. The suitable candidate will have substantial knowledge and experience in working with challenging behaviours and mental health disabilities. This casual position will require you to also deliver direct support to our clients. The current team leader works between 12-25 hours a week to meet client requirements.
This role includes, but is not limited to:
Provide direct support to monitor the clients' progress towards their goals. These include assisting activities of daily living, assist with medication, school drop off/pick up, and attend medical and therapy appointments;Arrange team rosters to meet client requirements and respond to booking change requests raised by clients and team members;Research and arrange suitable after school and school holiday activities to promote clients' independence and well-being;Respond to incidents and ensure accurate and timely incident reporting by individuals involved;Represent Chakra Care in the multidisciplinary team meeting and be the conduit for integrated support;Maintain accurate records and reporting on client progress and any concerns with assistance from the operations team;Troubleshoot as required.Ideal Candidates We are looking for individuals with:
Empathy, patience, and a genuine interest in people;Track record in disability support, community services, or a related field;Demonstrated leadership skills and the ability to manage and motivate a team;Excellent communication, problem-solving, and decision-making abilities;Knowledge of relevant regulations and best practices in working with individuals living with disability;Experience working in multidisciplinary teams;Above average word processing and computing skills;Ethical approach to challenging boundaries encountered in individual support.Essential Qualifications Valid Working with Children CheckAustralian police clearanceRight to work in AustraliaNDIS worker's screening checkA fully licensed and insured vehicle with 24hr roadside assistanceMinimum of 2 years' full time equivalent experience in direct individual supportTeam leader experience well regarded but not essentialIf you feel excited about this opportunity, we would like to hear from you!
