We specialise in assisting older adults, individuals with disabilities, those experiencing mental health challenges, and people at risk of and experiencing homelessness.
We are recruiting to deliver the Aged Care Service (ACAS) and invite skilled clinicians, experienced assessors, or passionate support professionals to join our dedicated team and help us make a real difference in people's lives.
ABOUT THE ROLE The purpose of this position is to lead a new Footprints Aged Care Assessment Services team.
KEY ROLE ACCOUNTABILITIES Program Management Drive the implementation of the service in relevant aged care planning region(s).Manage the program to ensure successful compliance with funding guidelines, contracts, and legislation.Strong oversight of KPI performance and achievement of service delivery team.Ensure contractual requirements are met as per the Department of Health and Aged Care contractual requirements.Manage any reporting and technical requirements in the My Aged Care portal.Support the reforms within Aged Care legislation, helping employees understand these reforms and implications for service delivery and clients.Investigate client complaints and other feedback, including escalation and management of such in line with internal policies/procedures and regulatory/contractual requirements.Reporting, investigation, and management of program incidents in line with internal policies/procedures, including escalation and reporting in accordance with regulatory/contractual guidelines.Develop and maintain sustainable networks and partnerships with key stakeholders and groups to enhance program and services referral base and facilitate successful outcomes for both clients and Footprints.Maintain oversight of internal and external communications which impact the program (e.g., changes to policies/procedures, internal and external reform implementation, etc.), and ensure effective communication of same to the team.People Management Provide leadership, coaching, training, and support to team members ensuring a collaborative approach in implementing and achieving seamless program and service delivery.Provide operational and professional leadership, promote best practice, and enhance the capacity of teams to provide high quality, consistent, and effective services that promote individual choice, wellness, and independence.ABOUT YOU To be successful in this role, you will need: Tertiary qualifications in a health-related discipline directly related to health, aged care, or related specialist area (e.g., registered nurses, medical officers, occupational therapists, physiotherapists, and social workers).Current unrestricted registration with AHPRA or be part of or eligible to be part of a relevant professional association.Demonstrated understanding/experience of the provision of community-based support services for the aged which reflects a person-centred approach that places clients at the centre of care with a focus on dignity, respect, choice, and self-determination.Demonstrated knowledge and experience in client-led support reflecting a person-centred approach, incorporating assessment, case planning, monitoring, and evaluation processes.Demonstrated knowledge and understanding of Aged Care legislation and current reforms.Demonstrated leadership experience and the ability to work in a team, with a focus on employee support/development and performance management.Demonstrated knowledge and/or experience in working with a diverse client group including people who are ageing, people experiencing mental illness, people experiencing homelessness, and people with disabilities and their carers/families.Demonstrated experience in community networking and working collaboratively with external service providers to plan and provide consistent, responsive, and appropriate client-focused services.Demonstrated experience in community and government networking to promote partnerships and organisational growth/capacity to respond to reforms and new opportunities.WHY Footprints?
