Team Leader - Aged Care Assessment ServiceUniting Communities is seeking self-motivated people to join our new Aged Care Assessment (ACAS) team! As the Team Lead, you will oversee a team of Home Support Assessors responsible for conducting thorough assessments of clients' needs, ensuring they receive the appropriate support and services to enhance their quality of life.
You will provide guidance and leadership to a team of assessors, ensuring they deliver high-quality, person-centred assessments, while also managing caseloads, ensuring compliance, and fostering a positive team culture.
This is a hands-on role with a strong focus on leadership, quality assurance, and continuous improvement.
The role responsibilities include (but are not limited to): Lead, mentor, and inspire a team of home support assessors to ensure high performance and a positive, collaborative team culture.Conduct performance reviews and provide guidance to foster professional development and team cohesion.Oversee the completion of client assessments, ensuring accuracy, timeliness, and compliance with relevant standards.Conduct regular audits of assessments and provide constructive feedback to enhance the quality of service delivery.Collaborate with external partners and internal teams to ensure seamless continuity of care and improve overall service delivery.To succeed in this role, you will need: Experience in home support services, with a strong understanding of needs assessments and person-centred care plans.Excellent communication and problem-solving skills.Commitment to quality and compliance, with a collaborative approach to team management.You also will need the following: Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist, or Social Worker.Current AHPRA Registration or eligibility for AASW Membership.Hold or be willing to undertake MAC Learning Certification and additional training modules as required.Knowledge of the Home and Community Care Sector.Understanding of and a commitment to the 'Positive Ageing' philosophy incorporating a wellness approach.About Us Uniting Communities is a leading, inclusive not-for-profit community services organisation working alongside more than 80,000 South Australians each year to reduce inequality, improve wellbeing, overcome disadvantage, and support people to live the best lives they can.
