We are looking for a Team Lead  to join our new and exciting care coordination program in Bathurst or Dubbo, NSW. 
The vacancy is ongoing, and will work Full Time, 75 hours per fortnight. Part Time hours may also be considered for the right candidate.

Assist in implementing a new program that will deliver care and services for people experiencing chronic disease and complex health issues Lead and support effective, efficient, and high-quality care coordination services Support improved management of chronic disease and complex health care needs Provide care coordination, primarily via telehealth, and follow the client's journey throughout their care Guide self-management and health literacy, empowerment and integration with general practice and key stakeholders. Tertiary qualifications in social work, nursing, human services, community services or health Registration with AHPRA or membership with the equivalent professional body Demonstrated operational and professional leadership and management experience Extensive experience in delivery of integrated health care Ability to comprehensively review, assess and develop care pathways for service participants Proven verbal, written, interpersonal and communication skills with the ability to adapt services to meet varying needs Ability to work with a diverse range of stakeholders to develop partnerships and service pathways.
