The TCP Coordinator will work closely with our Regional Manager to grow our business throughout Victoria and the aged care sector.
The Role Programmed Care is the community and in home direst care division of Programmed Health Professionals.
We are seeking and experienced Registered Nurse to join the team and the Transition Care Program (TCP) Coordinator.
The TCP Coordinator aims to improve health outcomes and assist clients to remain living at home following a hospital stay.
The Coordinator will provide safe, responsive and cost-efficient coordination of care services in line with the national Transitional Care Program guidelines and the hospital TCP team.
The Coordinator is responsible for the initial establishment and/or review of the TCP care plan following a comprehensive standardised assessment.
The role also includes coordination of direct client care of TCP program recipients, highlighting the need for referral to multidisciplinary or specialty services.
And to assist the client to set realistic health care goals.
