The Community Coordinator position is a busy and varied role responsible for building, supporting and strengthening social, family & community networks.
You will provide oversight of your clients' care plans, undertake assessments and develop goals to assist your clients to live safely & independently in our community.
You will be working with a supportive & collaborative team in a role which enables you to work with purpose each and every day, to really make a difference in people's lives.
In addition to the above, you will be responsible for:
Working with the team to deliver high quality services; Liaising with other service providers; Working with clients to community care programs including Home Care Packages, Commonwealth Home Support Program, NDIS, and ICare; Case management; Development of Day Programmes; Supporting staff in their role to deliver and maintain high standards of care. You are a strong leader with previous leadership and people management experience, bringing your business acumen to drive results through sales and marketing strategies and have had previous experience in a similar role or within a similar industry so you are aware of the complexities of the Aged Care industry, its compliance and clinical aspects of the role.
This will be a permanent position based in our Tamworth office, but you will be expected to be out and about as part of your role, meeting clients & attending networking opportunities.

Qualification(s) and experience in Community Care Coordination or be a Registered Nurse with AHPRA (Not essential); Current Class C driver's license; Proficient computer skills; Current NDIS Worker Check - NDISWC (or willing to obtain).
