About the role This is a one-year fixed term full time position with the possibility of extension within a purpose-driven team.
The role requires travel for outreach to Beverley; Cunderdin; Dalwallinu; Dowerin; Goomalling; Northam; Quairading; Tammin; Toodyay; Victoria Plains; Wongan-Ballidu; and York. Reporting to the Team Leader, Supportive Outreach, the primary responsibility of the Supportive Outreach Coordinator (SOC) is to provide specialist and intensive assistance to help people within the Care Finder target population to understand and access aged care and connect with other relevant supports in the community.
The role will include approved travel within your designated service delivery area and involves phone and direct (face to face) client contact to deliver information and support coordination.

Advanced knowledge of My Aged Care systems and processesDemonstrated understanding of the aged and community care sectorSound knowledge of holistic client service support networks and organisationsExcellent written and verbal skillsWell-developed facilitation/ negotiation skills in working with clients and engaging support servicesConfident presentation/ public speaking skills to deliver group education sessionsCommitment to delivering quality customer service, working empathetically with clients to ensure we deliver rights-based and client-focused servicesThe ability to work collaboratively within a small team where knowledge sharing is a key focus to improve outcomes for the older people we supportCan work autonomously in the community whilst practicing safe outreach activitiesStrong computer literacy, including Outlook, Word, PowerPoint, and client data management systemsAbout us Advocare is a leader in addressing the rights of older people and elder abuse.
