About the role This is a one-year fixed term full time position with the possibility of extension within a purpose-driven team. The role requires travel for outreach to Beverley; Cunderdin; Dalwallinu; Dowerin; Goomalling; Northam; Quairading; Tammin; Toodyay; Victoria Plains; Wongan-Ballidu; and York. Reporting to the Team Leader, Supportive Outreach, the primary responsibility of the Supportive Outreach Coordinator (SOC) is to provide specialist and intensive assistance to help people within the Care Finder target population to understand and access aged care and connect with other relevant supports in the community. The role will include approved travel within your designated service delivery area and involves phone and direct (face to face) client contact to deliver information and support coordination.
