What you'll be doing in this role We require qualified aged care workers to care for clients who are transitioning out of the hospital and into theNorthern Sydney Local Health District's Transitional Aged Care Program.
In this role, you will empower independence and improve the quality of life for clients living in theNorthern Sydney Area as you provide practical support and companionship for older people returning home after a stay in the hospital.You'll be required to travel to client's homes and communities using your own car.
Key Responsibilities Help at Home: Housekeeping, grocery shopping, and meal preparation. Personal Care: Assisting with personal hygiene, showering, dressing, mealtime, and medication administration. Transport: Driving clients to appointments or on shopping trips. Social and Wellbeing: Accompanying clients on outings and social support activities. Qualifications and Experience Certificate III in Aged Care, Individual Support or equivalent (preferred). Experience in providing client-centered care. Current First Aid certificate. Valid driver's license and access to a comprehensively insured vehicle.
