Role Qualifications Certificate III Aged Care /Individual Support/Disability or equivalent Completion of Infection Prevention and Control Training Medication Assistance Competency (where applicable) Manual Handling Certificate Current Basic Life Support (BLS) Certificate Holds current CPR and First Aid Certificate Hand Hygiene Certificate Possesses a NDIS Worker Screening Check Not more than 3 months Police Check Working with Children check (where applicable) Immunisation Record (Covid Vaccine/Yearly Flu Vaccine or other required immunisation Record) Driver's Licence (must be open to travel interstate) Skills Demonstrated knowledge of safe work practices Good time management skills and ability to follow care plans Good interpersonal skills and empathy for the elderly/disabled Skills in fostering rapport and relationship with clients and family Roles and Responsibilities Assist with daily living activities such as bathing, dressing, grooming, and toileting. Encourage and assist clients to achieve maximum independence in their daily activities. Ensure timely and accurate administration of prescribed medications. Develop and maintain trusting relationships with clients. Engage in meaningful activities and conversations to support emotional well-being and help clients stay connected with family and friends. Observe and report any changes in clients' health conditions. Help clients with mobility aids and exercises to maintain physical health. Collaborate with Community RNs and other health professionals to ensure comprehensive care. Assist with cleaning, laundry, and maintaining a safe living environment. Plan and prepare nutritious meals according to clients' dietary needs and preferences. Help with shopping, errands, and other household chores. Accurately document care provided and clients' progress. Communicate any concerns or incidents to the Community RN or Coordinator promptly. Adhere to individualized care plans and update them as necessary. Participate in mandatory training sessions and continuous education opportunities. Role Adherence Generate and maintain a trusting relationship with clients which includes protecting confidentiality, privacy, individual choice, and decision making. Implement a focused practice ensuring diversity and equity in service delivery, accommodating any special, cultural, or linguistic needs that have been identified. Communicate effectively and appropriately with clients, their families, friends, and allied health professionals. Adhere to Nursing Experts Services Policies and Procedures based on Australian Government standards.
