The Reconnect Programs support young people between 12 and 18 who are at risk of becoming homeless, or who are already homeless, or experiencing family conflict, living in Broken Hill, Wilcannia, Ivanhoe, & Menindee.
As a Youth Support Worker, you will provide early intervention case management support to young people at risk of homelessness, and their families. This includes the provision of individual case management, family support, group work programs, and provision of information and supported referral.
Due to the geographical area the program covers, outreach work is an essential part of the role; a pool car will be provided for outreach work.
Your key responsibilities will be to: Respond to referrals or approaches to the Program.Schedule and conduct ongoing case management sessions with whole families or individuals.Develop quality relationships with a range of key stakeholders.Develop and conduct a range of groups and information sessions for families.Participate in a range of interagency meetings, steering committees, and other forums.Accurately record information in management files for all clients in line with Mission Australia protocols and program.Proactively raise concerns that affect the safety and wellbeing of children and young people engaging with MA services.Qualifications & experience Tertiary Qualification and/or relevant experience in the human services industry desirable.Knowledge and experience working with issues relating to youth homelessness including domestic violence, substance abuse, and mental health.Demonstrated experience in working therapeutically in partnership with young people and their families.Demonstrated experience in facilitating groups.First Aid Certificate or willingness to obtain.Current NSW Driver's License.Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
