The Home Care Package

(HCP ) team provides an individually planned and coordinated suite of support and services designed to assist older people who need support to remain living independently in their own homes. Casual

position covering the area spanning from Laurieton to Kempsey (office based in Port Macquarie). MUST be living in this LGA. Open to experienced and new Support Workers with a minimum Cert III in Aged Care.

25% casual loading + Not-for-Profit Salary Packaging to maximise your take home pay.

We are looking for a Support Worker to work casually across our Home Care Packages program. Home Care Packages responsibilities include: Deliver high-quality individualized support and assist our older adult clients, supporting them to maintain and develop their independence so that they can lead enriching and fulfilling lives. Follow and implement individual service plans, healthcare, and positive behaviour support plans. Provide companionship, conversation, emotional support, and assistance with personal care in the client's home. Assist with transportation to attend appointments.

Who we are looking for: If you have experience working as a Support Worker, are qualified with a minimum Cert III in Aged Care, Community Support, or a related field, and enjoy variety in your day, you might be exactly the person we're looking for! Minimum Cert III Qualification in Aged Care, or Individual Support (must have completed aged care components). Experience working with older adults. The ability to follow and implement individual service plans, healthcare, and positive behaviour support plans. A desire to use your skills and experience creating fun and interesting activities that support older adults to engage more consistently with their community and peers. A valid driver's licence.

Qualifications Minimum Cert III Qualification in Aged Care, or Individual Support (must have completed aged care components).
