6 months contract role with possible extension Exceptional working conditions Challenging and very rewarding industry About us Ameya Care Support Services is a registered and dedicated NDIS provider passionate about delivering personalised support to individuals with mental and physical disabilities.
About the role As a Support Services Coordinator, you will provide responsive support coordination and individualised services for each participant consistently with the relevant legislation, their NDIS plans and Ameya Care Support Services policies, procedures and guidelines.
Working under the general direction of the Support Services Manager, your key areas of responsibility will include:
Maintaining written program documentation as outlined in service policy and standards guidelines, consistent with the Disability Act and the NDIS Act Investigating and compiling complete case notes and other records to ensure factual based reports are being delivered within strict timeframes Accurately entering and maintaining reporting data on the relevant management systems Qualification and Experience Current unrestricted C Class Drivers Licence Comprehensive Car Insurance National Police/Criminal History Check Working with Children Check/Child Safety Screening (Blue Card) NDIS Worker Screening (Yellow Card) Current Assist Clients with Medication (HLTHPS006) or equivalent qualification as approved by Ameya Care Support Services Current CPR Certificate (HLTAID009) or equivalent qualification as approved by Ameya Care Support Services Current First Aid Certificate (HLTAID011) or equivalent qualification as approved by Ameya Care Support Services Manual Handling Training Certificate NDIS Orientation Modules NDIS Induction Modules NDIS supporting effective communication module NDIS safe and enjoyable food module Food Handling Certificate Infection Control Training Certificate Valid Car registration Certificate Certificate III in Individual Support Industry Relevant Qualifications How to apply Please submit your current resumé together with a cover letter to ****** .
