Who are we looking for?We are seeking a dedicated and compassionate Support Facilitator who is passionate about empowering young people with lived care experience as they transition to independent living. You will have a strong ability to build trust and rapport with young people aged 18-25, offering practical support and guiding them to develop essential living skills. Using trauma-informed case management and support coordination, you will guide young people to access the resources and services they need, while fostering their independence and confidence.
What can you expect to be doing?Facilitate restorative, client-led decision-making processes with individuals and families.Provide trauma-informed case management and support coordination to achieve client goals, including assessments, goal setting, and casework.Conduct holistic screenings and risk assessments using tools like the Detection of Overall Risk Screen (DOORS).Develop, implement, and monitor individualised client objectives using required tools and frameworks.Offer referrals and support to build independent living skills, such as financial literacy, housing access, tenancy management, social connections, and respectful relationships.Serve as a central point of contact for clients and services, ensuring transparent and effective coordination, including access to specialist support when needed.What do you need to bring?Bachelor's Degree in Social Work or similar.Minimum two years' experience in a similar role.Experience in managing complex client matters and providing effective support.Proven experience in delivering trauma-informed practice.Experience working with Aboriginal and Torres Strait Islander young people, families, and communities, using culturally responsive approaches and processes.What we offer:Flexible work arrangements: Design your work-life balance.
