Empowering individuals to achieve their personal goals and enhance their quality of life Providing crucial support to families, alleviating caregiving pressures Ensuring efficient use of NDIS resources to maximize benefits for individuals Key Responsibilities As a Support Facilitator/Case Manager at RCS, you'll be at the forefront of providing comprehensive support to people with disability. Your key responsibilities will include:
Engaging in evidence-based practice for case formulation, assessment, reporting, and interventions Managing a long-term caseload, meeting KPIs and ensuring quality care Making strategic referrals to allied health professionals and service providers Building and maintaining productive relationships with individuals, stakeholders, and service providers Expertly managing individuals NDIS plans within allocated budgets Developing and providing timely reports on outcomes and success indicators Creating tailored, person-centered strategies aligned with funding guidelines Guiding individuals through NDIS processes and accessing community supports Maintaining accurate and confidential records in compliance with privacy standards Providing excellent customer service and direct support to individuals and their families Advocating effectively for individuals as part of a multidisciplinary team Completing implementation, progress, and annual reports for individuals Coordinating support services in accordance with NDIS funds and budgets Qualifications and Experience The ideal candidate will possess:
Minimum 3 years of experience in disability services or case management Degree-level qualification in a relevant field (social work, psychology, allied health, nursing, or community services) In-depth knowledge of the NDIS framework, Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Assisted Daily Living (ADL) and Community Access (CAS) Experience working with individuals with complex disabilities and high-intensity care requirements Strong leadership, communication, and relationship-building skills Exceptional organizational abilities and time management skills A compassionate approach coupled with a commitment to professional standards Highly developed verbal and written communication skills Comfort with technology and ability to work both autonomously and as part of a team Blue card and NDIS Workers Screening Clearance (or willingness to obtain) Current Australian Driver's License What We Offer At RCS, we value your contribution and offer:
