That's truly rewarding work that will help us continue to deliver life-critical services where it matters.About the roleThe Support Coordinator will manage a portfolio of severely injured clients who are at varying stages of their rehabilitation in the community setting, aligning the provision of medical, rehabilitation and disability services.Support Coordinators plan with clients to achieve optimal client outcomes and client experience in line with contemporary disability principles, and practice with a specific focus on health, social and work participation. This role you will spend a high percentage of your day engaging with clients, employers and treating practitioners.This role at times will require you to travel for face-to-face visits with clients and potentially their treating teams, therefore you will need to hold a current Victorian driver's licence.This role will be part of our Independence Branch, within the TAC. You will be supported by a manager who is open and flexible in their approach, your wellbeing and ability to succeed is paramount to the team's overall success. Working closely with a small team of Active Support Coordinators, you will join a collaborative team who is passionate about ensuring the best results for TAC clients.The Support Coordinator specialty areas include:The Intensive Support Coordinator manages a portfolio of severely injured clients, on a short to medium term basis, who are currently experiencing highly complex individual situations. Your clients will be at varying stages of rehabilitation and will often have additional factors affecting their recovery, so you will be required to coordinate multiple stakeholders and medical, rehabilitation and disability organisations, to ensure they achieve maximum independence and participation in all life areas during this time.The Early Support Coordinator proactively manages a portfolio of severely injured clients in the early stages of their claim, to achieve optimal outcomes. This is achieved through early hospital involvement, familial engagement and facilitation of discharge, the development of holistic Independence Plans, effective communication with stakeholders and sound claims management.The Active Support Coordinator will manage clients at varying stages of their rehabilitation in the community setting, aligning the provision of medical, rehabilitation and disability services with the client's individualised plan to achieve maximal independence and participation in all life areas. In the lifecycle of this role you will spend a high percentage of your day being face to face with clients, employers and treating practitioners.What you'll need to succeedQualifications and/or experience in a relevant industry, including care coordination, outreach services, social work/case management, mental health, allied health, disability or rehabilitationSound knowledge of disability support servicesResilience, an inquisitive mind, and a passion for making a differenceUnderstanding of medical, rehabilitation and disability principles and protocols, and the physical and psychological effects of acquired injuriesAbility to recognise and manage sensitive issues concerning clients and their familiesGreat communicator and can tailor communication style to suit different audiencesWhat we're looking forExperience in a client services environmentAssess client needs and develop plans in relation to a biopsychosocial model. Clear communication with the client pertaining to the goals of the plan and the scope of practice that TAC will support alongside formal and informal supports for non-motor vehicle accident-related needs.Put the client at the centre of decision making aligned with client goals and the Client Service CharterContinuously review the effectiveness of service provision and work with the client to adapt their recovery plan to ensure desired outcomes are achievedEmpower the client to choose the most appropriate services to meet targeted needsEmpathyUse empathy and develop rapport to engage in meaningful conversations with clients, utilizing motivational strategies to support them on their recovery journeyStakeholder EngagementProfessionally engage with stakeholders to collaborate and facilitate successful client outcomes and programs to aid in the client's independence journeyUtilise a claims management system and other systems to enable effective sourcing and storing of information.
