As our new Support Coordinator / Recovery Coach, your main focus will be to work with and support participants with psychosocial disabilities in their recovery, as well as coordinate supports in line with the participants' NDIS plan and with their individual goals.
You will partner with those in your care to strengthen their ability to design, manager and, ultimately, control their supports, assist them to take more control over their life and to better manager any challenges they encounter in day-to-day living.
This role combines recovery and support coordination to help participants develop relationships and build support networks to ensure they are fully supported throughout their recovery journey.
