As a Support Coordinator, you'll play a vital role in helping participants achieve their goals by facilitating access to services and community networks.
Working closely with participants, their families, and our multidisciplinary team, you'll connect individuals to the resources and supports they need to thrive.
Minimum Qualifications:Bachelor's or Master's degree in Social Work.Demonstrated commitment to the safety and well-being of children, young people, and people with disabilities.Strong organizational skills with proficiency in record-keeping, report writing, and Microsoft Office Suite.Effective interpersonal communication skills, including the ability to convey complex information in plain English to diverse audiences.Previous experience as an NDIS Support Coordinator.Current NDIS Worker Screening Check, Working with Children Check, First Aid certification, and driver's license (or willingness to obtain these).Key Responsibilities:NDIS Plan Coordination: Assist participants by managing their NDIS plans, negotiating services, and connecting them with community and allied health support.Client Empowerment: Promote a client-centered approach that supports participants in making informed choices and building resilience within their support networks.Community Engagement: Participate in networking events to promote Oyster Care's services, fostering strong community partnerships.Professional Collaboration: Work with our multidisciplinary team and other professionals to ensure holistic support for each participant.Timely Planning: Create intervention plans within two weeks of referral, supporting participants through plan reviews and discharge processes.Case Management: Maintain cases of participants, ensuring high-quality service and accurate, timely documentation.Schedule:Afternoon shiftDay shiftMorning shiftPublic holidaysExperience:Support Work or other relevant: 2 years (Preferred)Licence/Certification:Working with Children Check (Preferred)Driver Licence (Preferred)Work Authorisation:Australia (Required)Ability to Commute:Ability to Relocate:Newcastle, NSW: Relocate before starting work (Required)About Oyster CareOyster Care is one of the largest and most trusted not-for-profit providers of aged care in Australia.
