About the role In the role of Support Coordinator - NDIS, you will assist NDIS participants to coordinate and implement a range of support services, in line with their NDIS plan, using a person centred active support approach. You will establish strong rapport and relationships with a range of stakeholders internally and externally. This role is a six month maximum term, full time contract where you will have the flexibility to work from home and our Darwin office. There will be travel across Darwin required to meet with clients.

Key responsibilities Strengthen and enhance our client's ability to coordinate supports and participate in the communityRegularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budgetEnsure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequencySkills & experience Tertiary qualification - Diploma in Disability; or Certificate III or IV in Disability or equivalentDemonstrated experience in working with people with a disability and mental health issuesExcellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDISStrong written and computer skills for the creation of plans, programs reports, correspondence and other documentsEstablished time management, problem solving and conflict management skillAbility to think creatively and use initiativeSuccessful candidates will be required to clear probity checks including National Criminal History Record Check, NDIS Workers Screening Check and Working with Children Check.
To be considered for and appointed to a position at Life Without Barriers, you must be fully vaccinated against COVID-19 with a minimum of two (2) doses or hold a valid medical exemption certificate, in accordance with Life Without Barriers Employee COVID-19 Vaccination Policy. You will be required to provide evidence of your vaccination status during the recruitment process
