About the role Assist NDIS participants to coordinate and implement a range of support services, in line with their NDIS plan, using a person centred active support approach. You will establish strong rapport and relationships with a range of stakeholders including People with Disability and/or mental health issues, their family members, NDIS Local Area Coordinators, NDIS Planners, internal and external service providers.
This role is permanent part time (22 – 30 hours per week) where you will have the flexibility to work from home and/or from our Newcastle office.There is flexibility in how you complete the hours, whether you prefer shorter days across Monday to Friday or longer days 3-4 days per week, we are open to see what will best suit you. There will be some travel required to meet with clients across the Hunter region so a NSW driver's licence and your own reliable vehicle is required.

Key responsibilities Strengthen and enhance our client's ability to coordinate supports and participate in the communityRegularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budgetEnsure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequencySkills & experience Demonstrated experience in working with people with a disability and mental health issuesOperational knowledge of the NDISThe ability to work in a KPI driven environment where motivation and commitment are essentialExcellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDISStrong written and computer skills for the creation of plans, programs reports, correspondence and other documentsAbility to think creatively and use initiativeSuccessful candidates will be required to clear probity checks including National Criminal History Record Check, NDIS Workers Screening Check and NSW Working with Children Check.
To be considered for and appointed to a position at Life Without Barriers, you must be fully vaccinated against COVID-19 with a minimum of two (2) doses or hold a valid medical exemption certificate, in accordance with Life Without Barriers Employee COVID-19 Vaccination Policy.
