About the role Assist NDIS participants to coordinate and implement a range of support services, in line with their NDIS plan, using a person centred active support approach.
You will establish strong rapport and relationships with a range of stakeholders including People with Disability and/or mental health issues, their family members, NDIS Local Area Coordinators, NDIS Planners, internal and external service providers.
This role is a permanent part time (three days per week where the days are flexible) opportunity where you will have the flexibility to work from home and/or from our Hobart office.
There will be some travel required to meet with clients across the Hobart region so a TAS driver's licence and your own reliable vehicle is required.
Key responsibilities Strengthen and enhance our client's ability to coordinate supports and participate in the communityRegularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budgetEnsure mainstream services meet their service obligations to clients (i.e.
housing, education, justice, health)Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequencySkills & experience A genuine interest and commitment to partner with people with a disability and/or mental health issues to change lives for the betterThe ability to work in a KPI driven environment where motivation and commitment are essentialExcellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDISStrong written and computer skills for the creation of plans, programs reports, correspondence and other documentsAbility to work autonomously and use initiativeSuccessful candidates will be required to clear probity checks including National Criminal History Record Check, NDIS Workers Screening Check and Working with Vulnerable People registration.
To be considered for and appointed to a position at Life Without Barriers, you must be fully vaccinated against COVID-19 with a minimum of two (2) doses or hold a valid medical exemption certificate, in accordance with Life Without Barriers Employee COVID-19 Vaccination Policy.
