Two days per week you will be working from our Hornsby office, however work-from-home options are available for the other three days.
You will be required to support our clients to participate and develop valued roles in the community, to make their own decisions about how they live their life and facilitating the support they want for their life.
The responsibilities of the position include: Assisting clients to connect to and engage with informal and mainstream funded supports which will assist the client to achieve their goals.
Supporting clients to access supports according to the funding provided in their NDIS allocated budget.
Fully understanding the environment in which the client is living and being able to respond to and resolve crisis circumstances that impact on the client's ability to achieve their goals.
Building capacity in clients to preserve existing support relationships with the end goal being a decrease in client's need for support connections.
Building and sustaining relationships with external service providers and mainstream supports for the purpose of maximising opportunity for clients.
Preparing and maintaining all relevant administration records, systems, reports, and general documentation in accordance with Unisson Disability's policies and procedures.
OUR IDEAL CANDIDATE: Experience working within the National Disability Insurance Scheme (NDIS) and an understanding of the complex needs of people with disabilities.
Ability and willingness to work flexible hours.
