We specialise in providing professional in-home care services for participants under the National Disability Insurance Scheme (NDIS) and My Aged Care.
Through level 2 and 3 support coordination, we assist individuals in achieving their identified plan goals and help them gain access to the needed programs and support services, with a multidisciplinary one team approach.
Job Description The role of the Support Coordinator is to provide support in specific high-risk situations. This support can be time-limited and aims to address barriers and reduce complexity in the support environment while assisting the participant to connect with support and build capacity and resilience. It may also involve the development of an intervention plan which will be put in place by Disability and/or Mental Health Support Workers. This scope also includes both Level 2 Support Coordination and Level 3 Specialist Support Coordination.
Responsibilities include but are not limited to:
Accept referrals and engage NDIA participants and their network.
Engage with individual participants and collaboratively implement their NDIS plan.
Provide assistance linking participants to appropriate providers including, but not limited to, Spectrum Care to meet the needs identified in their NDIS plan.
Have a collaborative team work ethic and also be able to work independently to ensure continuity of care and provision of a comprehensive service to participants.
Coordinate a range of supports and services including informal, mainstream, and funded providers.
Collect, collate and maintain data on all participant interactions, ensuring that the data is accurate, up to date, and complies with reporting guidelines.
Collect information on local community services and supports.
Maintain participant records/case notes and statistical information in the system application.
Provide regular feedback and reports to NDIA as required.
Participate in the review of the plan as required and regularly report to management regarding the achievement of participants' individual plans.
Engage informal and formal support networks, and resolve points of conflict or crisis to develop participant's capacity and resilience within their own network and community.
Provide time-limited (specialist) support coordination to address high-risk situations and assist the participant to connect with support and build capacity and resilience.
Develop intervention plans for implementation by Disability and/or Mental Health Support Workers as required.
Comply with all Spectrum Care policies and procedures, sales strategy, and all WH&S procedures to ensure safe work practices.
Perform any other duties as required by Management that are within the candidate's skill level and capabilities.
Attend relevant professional development training as approved by your Manager.
RequirementsQualification in one or more of these courses: Certificate III in Individual Support (Ageing, Home and Community), Certificate III in Individual Support (Disability Services), Certificate III in Community Services, Certificate III in Aboriginal and/or Torres Strait Islander Primary Health Care, Diploma of Community Services.
Diploma of Mental Health, Diploma of Counselling.
