The Support Coordinator will work with the clients to implement the supports within the person's plan for the purpose of building capacity, managing the impact of their disability, and improving their health, well-being, and participation within the community.Reviewing NDIS plans and ensuring appropriate supports and services are implemented.Ensuring regular contact is maintained and recorded with clients.Managing client issues and complaints with respect and fairness whilst following policies and procedures.Prioritizing, managing, and administering a caseload/task in a coordinated, efficient, and timely manner.Coordinating supports for participants and liaising extensively with NDIA planners and a range of allied health and community-based services.Supporting the development of service agreements, bookings, referrals for assessments, and urgent equipment within the plan parameters.Enhancing participants' capacity to coordinate their own supports, including provision of information, referrals, and engaging with informal/personal support networks and community partners.Who we are looking for…We are looking for applicants with at least 2 to 3 years experience in an NDIS Support Coordinator role backed with relevant qualifications in disability or mental health.To be successful in the role you will have:Strong understanding of the NDIS framework and person-centred practices.Experience in the delivery of case management and coordination of client services.Ability to interpret NDIS plans and recommend service options within the guidelines of the plan and budget.Previous experience working in disability support or a related field.Proven experience working with people with autism, disability, and/or mental illness with Service Provision.Demonstrated experience in Client Directed Care environment with planning, support, and case coordination facilitation, including people with disabilities, mental health, and other significant support needs.Sound verbal, written communication, and negotiation skills.A current full driver's licence (essential).Qualifications & Attributes:Candidates with previous work experience or lived experience working with individuals with disabilities is highly advantageous.Certificate III or IV in Disability Support or a related field (or currently studying towards).Current NDIS clearance.Values that indicate a passion to help others, integrity, and honesty.Current National Police check or willingness to apply.Strong interpersonal and communication skills.Empathy, patience, and a genuine desire to make a positive impact in the lives of others.Ability to work independently and as part of a team.Understanding of the NDIS framework and principles.At YLDP we pride ourselves on giving our employees training to enhance skills and knowledge and a supportive leadership group.Should you be successful, you will be backed by a group of collaborative team members and a company that truly cares about your career and your future.Aboriginal and Torres Strait Islanders are encouraged to apply.If you would like to find out more about this opportunity, please APPLY.
