You'll work with children and adults living with muscle wasting conditions and develop lasting relationships with community members who rely upon you to help to maintain their independence and live the lives they choose.
This position is a 12 month, full-time contract providing Support Coordination, Specialist Support Coordination and case management under our charitable model.
No two days are the same at Muscular Dystrophy Queensland.
You will have a mix of visits to Participants' homes, as well as community meetings and online work related to implementation of their NDIS Plans.
You will be working within an extended team of allied health professionals and plan management staff.
Your manager and experienced colleagues will support you to upskill in neuromuscular practice.
