About the role
Agile Skills is seeking a dedicated Support Coordinator to join our team in Mackay, Queensland. As a casual Support Coordinator, you will play a vital role in helping older adults and people with disabilities access the care and support services they need to live independently and participate fully in their communities.
What you'll be doing Assisting clients to navigate the aged care and disability support systems and access appropriate services Developing individualised support plans in collaboration with clients, their families, and other stakeholders Coordinating and monitoring the delivery of support services, ensuring high-quality care and client satisfaction Advocating on behalf of clients to ensure their needs are met and their rights are protected Maintaining accurate records and documentation in line with organisational and regulatory requirements Collaborating with a multidisciplinary team of healthcare professionals to provide holistic support What we're looking for Relevant tertiary qualification in a health, social work, or human services-related field Minimum 1 years' experience in a similar support coordination or case management role Extensive knowledge of the aged care and disability support systems, funding schemes, and available services Strong interpersonal and communication skills, with the ability to build rapport and work collaboratively with clients, families, and service providers Excellent organisational and time management skills, with the ability to prioritise and multitask Compassionate and empathetic, with a genuine commitment to improving the lives of older adults and people with disabilities Option of Sub-Contracting or Direct Employment Flexible working from home opportunities.
