Support Coordinator

The Role:
As a Support Coordinator, you will work closely with NDIS participants to help them achieve the goals outlined in their NDIS plans. You'll be responsible for coordinating and engaging service providers, developing relationships with participants, and using a coaching approach to foster independence. The role involves managing participant intake and referrals, maintaining accurate case notes, and offering crisis support when necessary. You will also liaise with participants' informal supports, such as family and carers, while working collaboratively with the broader team. A critical aspect of this role is ensuring that services are delivered in line with NDIS guidelines and meeting the participant's needs. This position also involves assessing and managing risks in line with organisational policies, with a focus on delivering high-quality services to participants.
