Support Coordinator

Support Coordinators assist participants in finding providers for the support participants are funded for to allow individuals to achieve their goals and an independent life.
We offer flexible working arrangements for the right candidate in a dynamic work atmosphere that values multicultural diversity.ResponsibilitiesSupporting and empowering participants to make their own choices regarding how they want to lead their lives.Coordinating various services for participants to support them in living fulfilling and meaningful lives within the community.Engaging with and managing participants' key stakeholders (including government agencies, community, and clinical service providers and families) and ensuring that the supports and services are leveraged, not duplicated.Working with participants to strengthen their ability to coordinate their support to live their best lives.Supporting participants to establish their network and maintain support services, links, and relationships.Helping participants understand their NDIS plan, goals, and objectives whilst promoting choice and control.Preparing for Plan Reviews and attending with the Participant, Family, or Representative.Skills & ExperienceRelevant Tertiary qualifications in Community Services, Mental Health, and Disability or similar.Preferably 1+ year of experience in a similar role.Demonstrated experience with facilitating Multi-Disciplinary Coordinated Planning and developing collaborative relationships and networks between service providers.Extensive knowledge of mainstream and NDIS systems and practices.Excellent verbal communication skills and a demonstrated ability to engage with participants and their families.We wish to work with people who reflect the diversity of our participants to ensure we can support individual needs and wants.
