This position is a full-time position based in Ararat.
Your responsibilities will include: Providing advice and coordination services to participants to allow choice and self-management to ensure the required services are receivedPlan and implement services and supports that empower participants to participate in activities.Promote best practice 'person centred care'Provide participant services to address their wellbeingMaintain networks and knowledge of services and liaise with service providers.About you You will be an experienced passionate Support Coordinator with outstanding communication and client service skills allowing you to build strong relationships with a diverse range of people.
You will have excellent time management and organisation skills with the ability to manage competing demands to meet client needs.
