ROLE REQUIREMENTS: Allied health or community services qualification, such as Social Work, Nurse, Counselling, etc (or studying)Community services or allied health experiencePassion for the Disability and Community sectorExcellent written and verbal communication skillsExperience in note-taking and report writingCreative problem-solving skillsExcellent organisational and time management skillsAbility to create and maintain lasting professional relationshipsCaring, empathic, and understanding naturePositive and energetic team memberA strong sense of professional boundaries and integrityBrisbane based home to travel fromAutonomous and preference for working from homeCurrent Australian Driver's Licence and access to registered and insured vehiclePREFERRED BUT NOT ESSENTIAL: Understanding of the NDIS and government funding schemesCase management or NDIS coordination experienceKEY DUTIES: Supporting clients to understand and implement the funded supports in their NDIS plans.Working collaboratively with clients and their families to build up their capacity to manage the supports within their NDIS plan.Linking participants to suitable service providers and allied health professionals.Assess client needs and monitor progress toward the achievement of their plan goals.Connect with clients in the community and remotely.Identify community or mainstream supports that can also assist participants to achieve their set plan goals.Communicate with and develop professional relationships with stakeholders, including service providers, allied health professionals, NDIA, Queensland Health, Office of the Public Guardian, etc.OPPORTUNITY INCLUDES: Flexible arrangements (full-time or part-time available) and healthy work-life balance encouraged while working from home.Training, supervision, and professional development.Joining a supportive and fun team who shares common values.Fulfilling and meaningful community services position.Staff rewards and team building opportunities.APPLICATIONS: If you possess the skills, passion and dedication to be successful in this role, and truly make a difference to our client's lives, we would love to hear from you!
