Hours per fortnight: 40Permanent Part Time Opportunity to utilise your skills in care coordination, patient assessment and patient centred goal settingChallenging and rewarding role in a supportive environmentAn exciting opportunity exists for a suitably qualified Allied Health Professional to take the next step in their career and join the Rehabilitation Ward as a Subacute Intake Coordinator.The Subacute Intake Coordinator collaborates with the Bed Manager, Rehab NUM and Rehab Clinical Health Lead to develop and maintain streamlined intake systems for the Rehab Ward in the context of the broader intake system for bed based services at ERH. The role also has overall responsibility for assessing all referrals and coordinating admissions to the Rehab Ward. The Rehabilitation Ward combines contemporary inter-professional practices and fantastic patient outcomes in a brand new state of the art rehabilitation unit.The Rehabilitation Ward provides care for patients in four care types: Rehab, GEM, TCP and Palliative Care and therefore ensures a diverse and dynamic working environment.Key responsibilities include:Ensure the systems for Rehab Ward intake and transitioning patients to the Rehab Ward are streamlined, efficient and patient centred.Work collaboratively with the Bed Manager to ensure the Rehab intake system is consistent with other intake systems for ERH hospital bed based services.Ensure and support staff to use practices consistent with the ERH system for the referral of patients to bed based services and transitioning patients from one service to another.Actively input into the development of the Bed Manager KPIs suite to ensure the effectiveness of intake and transfer systems are routinely monitored and assessed.Coordinate the intake of individual patients to the Rehab Ward.Attend care coordination meetings.
