The role is to assist and support consumers, their families, and support networks in response to physical, emotional, and social factors that negatively impact on the consumer's health and wellbeing. The social work role also has a primary and secondary prevention component focused on averting the onset of poor mental and physical health and early intervention by addressing social and environmental determinants of health.Details of Appointment:Part-time, fixed term appointment until 30 June 2026, working up to 76 hours per fortnight across Daylesford, Creswick, and Clunes sites.
