About the Diabetes Education Service: The Diabetes Education Service at Austin Health provides high quality, best practice education and support for paediatric and adult patients living with diabetes, their family members, and support persons in hospital and in our outpatient clinics. We provide inpatient and outpatient diabetes education across the three Austin Health sites: Austin Hospital, Heidelberg Repatriation Hospital, and the Royal Talbot Hospital, as well as working closely with our community partners.
About the Role: We are seeking a Diabetes Clinical Nurse Consultant to work in a fixed term, parental leave cover position for 2 days a week (Thursday and Friday). This position is located within the Medical & Cancer Services Division. The Diabetes Clinical Nurse Consultant employed in this position will be based primarily at the Heidelberg Repatriation Hospital but will also provide services to paediatric and young adult inpatients and outpatients across both the Austin Hospital and Heidelberg Repatriation Hospital sites. Some off-site travel may be required.
Responsibilities: In this role, you will ensure patients with diabetes and their families receive optimal support and diabetes education and management both as inpatients and as outpatients. This includes:
Prompt and efficient review of diabetes patients as soon as possible after referral received Identifying knowledge gaps for patients and Austin Health staff and using this knowledge to develop educational programs accordingly Ongoing self-education and professional development in all aspects of diabetes education and management About You: You will be someone who has excellent communication skills and a strong commitment to excellence in patient care. You'll bring energy and drive to your work, and have a keen interest in developing your skillset. You'll further have:
Demonstrated diabetes knowledge and skills and ability to work as a sole practitioner High level communication skills - written, verbal, and interpersonal Paediatric diabetes education experience in an acute setting Understanding of latest diabetes technology systems; including insulin pumps and continuous glucose monitoring and flash glucose monitoring systems Benefits: Working at Austin Health means enjoying a strong sense of purpose, engaging in meaningful work every day.
