About the Role: The Staff Specialist will be part of the Residential Aged Care In-Reach (RAC-I) service with a specialised aged care focus.
The Staff Specialist will provide clinical care in collaboration with the Nurse Practitioner and registered nurses team, to maximise patient-centred care and quality of life by providing a proactive, integrated, specialised urgent care to Residential Aged Care Facility (RACF) residents within their usual accommodation.
Service provision will be face to face or via virtual care.
This RAC-I will sit alongside and complement other Hunter New England Local Health Care services that already support our older population.
By joining our team, you will play a vital role in providing high-quality, patient-centred care as part of the RAC-In Reach multidisciplinary team.
You'll deliver specialised urgent care services to residents in Residential Aged Care Facilities (RACFs) within their familiar surroundings.
This innovative model emphasises proactive, integrated care to reduce hospital transfers, ensuring residents receive timely and effective nursing and multidisciplinary support.
You'll also collaborate closely with RACFs and their usual GPs to ensure continuity of care and enhance the quality of life for residents.
Essential Requirements A primary medical degree registered with AHPRA and Fellowship of the Royal Australasian College of Physicians (Geriatric Medicine) Evidence of being a medical expert, demonstrated by recent and comprehensive experience in Geriatric Medicine.
