Role Status: Part-Time Term Vacancy until 02 July 2025, 30 hours per week Remuneration: AHP2 ($88,255 - $102,240 + 11.5% Super) / PO2 ($88,255 - $97,667 + 11.5% Super) Role Details: The Department for Child Protection is seeking a Stability Post Care Practitioner to join the Out of Home Care team on a part-time term basis.
The role is responsible for: Providing specialist assessment, support and referral service that meets the individual needs of clients, supports their family based care environment and assists in facilitating successful transition to independent living.
Enhancing family relationships in a culturally appropriate manner, including providing information and support to carers.
Providing professional guidance and support to liaison officers in the Stability in Family-Based Care (SFBC) program.
Undertaking specialist and comprehensive assessment of the needs of young people and their carers to assist in the development of SFBC Agreements, liaison with other services and referrals.
About You We are looking for confident individuals who have experience in working within the child protection and out-of-home care sector.
You will have sound knowledge of the needs of young people transitioning from care and experience in assessing their needs and facilitating access to life skills services such as, how to manage their finances, housing counselling, employment, and education/training.
Your strong interpersonal skills will assist in fostering and maintaining effective working relationships with people from diverse professional backgrounds and working as a team member to provide holistic professional practice.

Special Conditions: Essential: AHP: A degree level qualification in Social Work which gives eligibility for full membership of the Australian Association of Social Workers.
PO: Appropriate degree qualification in Community Services, Social Sciences, Human Services, Health or related field.
Persons of Australian Aboriginal or Torres Strait Islander descent, who have the appropriate background and skills but do not have the essential qualification, may apply for and be engaged/assigned to the role of Social Worker and will be entitled to apply for any Allied Health Professional roles requiring a qualification in Social Work within the Department for Child Protection (DCP).
