Your expertise will empower clients to overcome communication and swallowing challenges, increase independence, and reach personal milestones. You will work closely with clients, their support networks, and a dedicated team of professionals to deliver person-centered, holistic support that truly makes a difference.
Key Responsibilities Client-Focused Assessments: Conduct thorough assessments of clients' communication and swallowing needs, taking into account their unique goals, cultural background, and preferences.Tailored Interventions: Develop and implement individualized, evidence-based treatment plans to support clients in achieving their NDIS or Aged Care goals.Collaborative Support: Partner with clients, families, carers, and support workers to provide training on augmentative and alternative communication (AAC) systems and safe mealtime management strategies.Case Management: Actively participate in case conferences, care planning, and consultative meetings to foster coordinated and client-centered care.Accurate Documentation: Maintain timely and precise documentation of assessments, interventions, and progress to ensure transparency, accountability, and high-quality service delivery.Continuous Improvement: Engage in clinical supervision, professional development activities, and quality improvement initiatives to enhance both personal and team performance.The Ideal Candidate To succeed in this role, you should be passionate about person-centered care and comfortable working in a community-based or telehealth setting. You are someone who can balance empathy with professionalism, effectively manage time, and communicate with individuals from diverse backgrounds.
Qualifications: Bachelor's or Master's degree in Speech Pathology.Certification: Membership (or eligibility) with Speech Pathology Australia as a Certified Practitioner.Experience: Demonstrated skills in speech pathology services, including the assessment and intervention for communication and swallowing challenges.
