across our two day centres at Booragul and Belmont, with our Residential Aged Care sites and in home.
We are a dynamic team who work well together and provide exceptional support for each other and the people we support.Must be able to work Monday OR Friday along with a Tuesday, Wednesday OR Thursday.Role & Responsibilities:Delivering Speech Pathology services in individual and group settings focused on capacity building, maintenance, driving restorative outcomes, promoting healthy ageing, optimising quality of life, and reengagement with community activitiesConduct Speech Pathology assessment and intervention of swallowing and communication disorders using appropriate standardised, non-standardised and clinical assessment toolsProvide a high standard of clinical service that reflects and promotes evidenced based practiceSupport an appropriate case load effectively through planning, clinical prioritisation, and proactive time management skillsIdentify individual consumer's needs resulting in the development and implementation of a holistic therapy plan in consultation with the consumer, carer, relevant stakeholders, and frameworks (internal and external)Ensure plans are developed based on the goals, strengths and identified needs in consultation with consumers.
