Job overviewPlanned Therapy South Sefton service provides therapy and rehabilitation in patients' homes. We aim to promote independence and improve quality of life for people with physical problems caused by accidents, ageing, disease or disability. These include respiratory conditions, orthopaedic and musculoskeletal conditions, mobility issues, neurological conditions and those receiving palliative care.

The role involves working in Planned Therapy South Sefton team to provide support to our patients in their homes and other community settings, which will enable you to develop skills and flourish within your career. Main duties of the jobThe post holder will be experienced in management of complex conditions including neurological conditions, will be expected to effectively manage a caseload of patients' co-morbidities as an autonomous practitioner, using evidence-based patient-centred principles to assess, plan, implement and evaluate interventions for the patient group. The post holder will provide leadership for junior staff, when appropriate, through supervision and appraisal. The post holder will participate in planning, development and evaluation of services provided, holding responsibility for defined service improvement projects.
The post holder will take a holistic approach in the assessment and treatment of patients, being responsible for the provision of appropriate, high-quality therapy and care to a caseload which may be of a complex nature, using the appropriate model of care and evidence-based practice.
