Why this role mattersThe Salvation Army Australia's Family Violence Stream provides a wide range of services around the country to support women and women with children impacted by family and domestic violence. We offer refuge, children's and parenting services, counselling, support, men's programs, accommodation and advice to women and children who are experiencing, or who have experienced, family violence.About the roleWe are currently seeking a values-driven and proactive Specialist Family Violence Case Practitioner who can provide case management support to women, families and children who have experienced and/or are currently experiencing family violence within a refuge-based setting in Karratha. The role also encompasses the delivery of comprehensive risk assessment, safety planning and crisis responses. The ideal candidate will be instrumental in delivering high-quality, safe and effective family violence and child well-being responses to those seeking support, safety, and recovery.You will successfullyMaintain effective relationships with clients that encourage self-determination and participation.Provide case management support to clients in accordance with TSA FV Model of Care and Case Management model which meets the standards of best practice.Undertake ongoing risk assessment and safety planning.Deliver case management to address individual needs and circumstances from initial intake to exit.Develop individual case plans in collaboration with clients that include a range of interventions addressing the needs or goals of clients.Assist clients to access support services, establish community supports, and advocate with service providers when needed.Regularly monitor and review progress and outcomes of individual case plans, taking action as needed to ensure the plan continues to meet client needs or goals.This is a full-time position based in Karratha, WA. Salary and conditions are in accordance with the Social and Community Services WA Level 5 Award.
