The Role & Benefits:Based in Brisbane, the role will see you working with an established team from different professions, coordinating a range of support services in people's homes, community centres and other settings. We are looking for someone who can lead support workers in the field, and provide high level insights to complex barriers that often arise in this line of work. You will have a strong awareness of cultural barriers, limiting factors, relationship dynamics and the NDIS. Day to day, you can expect a great deal of diversity as you navigate highly individualised support pathways for each person and family member you impact.

Requirements:Tertiary qualifications in Social Work with current AASW registrationStrong experience with the NDIS and understanding of support coordinationCommunity Aged Care experience is a big bonusExceptional emotional intelligence, with the ability to respectfully work with people from all walks of lifeA genuine motivation for making a differenceA commitment to joining a loyal team that considers themselves a familyIf this opportunity is calling to you, we urge you to apply.
