Job Purpose - The Social Prescribing Link Worker (SPLW) is part of a pilot program funded by the Qld Government, aimed at addressing social isolation and loneliness. The SPLW empowers individuals to take control of their health and wellbeing by connecting them with community activities and services.
Key Responsibilities Community Engagement: Cultivate and maintain effective relationships with community stakeholders.Intake, assessment, relationship building and client management: Conduct holistic assessments of referred community members to understand their needs and facilitate connections to community services.Fostering positive team culture: Nurture a positive work environment that encourages innovation.Quality, risk and compliance: Participate in safety audits and ensure compliance with relevant frameworks.Governance and reporting: Support the maintenance of NCC policies and procedures and contribute to program evaluation.Key Skills, Experience and Qualifications Bachelors Degree or equivalent in social work, human services or similar.Excellent verbal and written communication skills.Experience working with diverse families, including First Nations and multi-cultural families.Knowledge of social prescribing and well-being.Strong understanding of the local community service system.Personal attributes Proactive and innovative problem solver.Highly organized and able to work autonomously.Strong interpersonal skills with a passion for community engagement.Other Must hold a current Blue Card.
