You'll manage your own caseload, using your professional judgment and problem-solving skills to navigate the intricacies of discharge planning and complex referrals.
Your day will be a blend of direct client interactions, case conferencing with multidisciplinary teams, and providing guidance to colleagues in managing complex social work cases.
You will lead family meetings and consultations, contributing to the development of care pathways and the continuous improvement of the Transition Care Program.
