We are looking for a Senior Social Worker to join us in a full-time capacity in the Women, Children and Family Division at West Moreton Health.
In this role you will apply a high degree of independent clinical judgement to provide clinical social work care where clinical needs are complex and evidence-based interventions are required.
The purpose of the role is to provide high-level, specialist interventions and related clinical services within Ipswich hospital Maternity, Special Care, Antenatal and Paediatric clinical services. This role will provide clinical practice, advice, and consultation to the multidisciplinary teams to enhance service delivery and support the continuum of care from hospital to the community. All clinical services are provided in line with the Social Work professional code of ethics and practice standards.
The role will utilise high-level knowledge and skills to undertake clinical service delivery, where clinical needs are complex and require assessment to identify clinical risk, high-level advocacy skills, and leadership to the clinical team on the utilisation of relevant legislation and policy to achieve safe patient outcomes.
About youOur ideal candidate will be someone who can demonstrate the following:
Demonstrated high-level expertise and skills in delivering assessment, interventions, and independent decision-making to provide evidence-based assessments and interventions for families with complex needs, with a focus on at-risk children and families and bereaved parents, ensuring a high standard of patient-centred and trauma-informed care.Demonstrated high level of professional leadership applied to a complex caseload of adult patients, children, and their families within an interprofessional team, whilst exercising a high degree of independent practice and professional decision-making, accessing clinical practice supervision and guidance as required.Demonstrated effective written and oral communication skills in providing multidisciplinary care in a complex environment including psychosocial assessments, management of clinical documentation, consultation advice, reports, and high-level advocacy to contribute to patient care.Providing a high level of clinical leadership in the supervision and development of staff including supporting effective and evidence-based practice to implement improvement in processes and active and ongoing participation in professional development.Providing clinical advice and applying clinical expertise to assist multidisciplinary care by effectively integrating the social work perspective and collaboration as required to improve health outcomes.West Moreton Health is one of the most rapidly evolving health services in the south east corner, with a community growth expected to almost double to more than 588,000 by 2036.
